During some random-ish Googling, I came upon an interesting Japanese Wikipedia entry. Apparently, in 1983, after the success of Ai no korida (In the realm of the senses) and Hakujitsumu (Daydream), Tetsuji Takechi made another big-budget film with real actors doing hardcore scenes. Oiran, whose IMDB page seems unimpressive and whose plot sounds like a zero-budget B-movie, is that film.
Getting a copy is fairly straightforward. Just google it and you'll find a plethora of sources, from Vimeo, through porn-sites and even torrents. Even this forum has a link. However, all the versions I managed to find are censored, heavily so, with hugh pink blots covering all of the action (and much of the plot...). The wikipedia page says this is the real deal. The original actress stepped down when she realizes "sex in front of the camera" meant actual sex in front of the camera, and her replacement went for it fully. She even agreed to a creampie, according to a later interview with her. And while this was her debut film, the rest of her career was that of a "normal" actress with no other sex film credits.
I doubt an uncensored version exists, because Japanese are really strict with their censorship. However, as far as I can gather, since this was only the 3rd sexually explicit real film released in Japan, and the 2nd such Japanese production (Ai no korida was a French production), it might have been originally released uncensored. Whether that's true, and whether that version survived for the past 40+ years is a different story.
